Mohs surgery, also known as Mohs micrographic surgery, is named for the physician who pioneered its use in the 1930. It is a tissue-sparing surgery that is microscopically controlled to resolve skin cancer as effectively as possible while minimizing the removal of healthy tissue.

Mohs surgery consists of cancerous skin cells being surgically removed layer by layer. After a removal, the tissue is examined under a microscope to check if the margins are clear of cancerous cells. If they are, the surgery is complete. If not, the surgeon returns to the patient and carefully addresses the affected area again. This is repeated until all cancerous cells are removed.

When will a doctor recommend Mohs surgery?

Mohs surgery is the treatment of choice for many common skin cancers, including basal cell carcinomas and squamous cell carcinomas. It is popular because it is effective, and also minimizes the aesthetic impact of the surgery on the patient.

Mohs surgery is often recommended specifically for treatment in delicate areas where aesthetics are concerned, such as the face or the backs of the hands. By removing the least amount of tissue possible, the appearance of scarring is reduced.

What should I expect?

If you are going to have Mohs surgery, plan on spending a long day at the dermatologists. Consider bringing something to entertain yourself during your wait, such as a book, and a snack.

In most cases, the surgeon will use a topical anesthetic to perform Mohs surgery. You will be conscious, but will likely feel little to no discomfort. After the surgery, the dermatologist will close or cover the wound. Depending on the size and location, you may have a plastic surgeon review or close the area.
